Telehealth
Telehealth is the consistent and accurate remote monitoring of patients' vital signs usually from their own home.
Used to manage chronic health conditions, telehealth is linked to a monitoring centre where the collected data is reviewed and those with abnormal readings for them, flagged for follow up. This allows the patients care team including community nurses, GPs and specialists to closely monitor the patient’s condition and intervene before it reaches crisis level and results in hospitalisation.
Telehealth is ideal for:

- Early hospital discharge
- Remote communities, where access to doctors is limited
